Pre-K/Kindergarten Round-up
F ebruary - March 2009

Admission is based on first come-first served basis and completed paperwork. 
You must register at your child's school
and have available on the designated registration day,
all required items to be considered for admission.

Registration will be held from 8:00 AM - 12:00 PM at the sites and dates listed below. 
Applications may be obtained and completed prior to registration from the school. 
Registration packets will be available at each school after February 3^rd.

CHILDREN MUST BE FOUR (4) YEARS OLD ON OR BEFORE SEPTEMBER 1, 2009
TO BE ELIGIBLE FOR ENROLLMENT IN PRE-K

CHILDREN MUST BE FIVE(5) YEARS OLD ON OR BEFORE SEPTEMBER 1, 2009
TO BE ELIGIBLE FOR ENROLLMENT IN KINDERGARTEN

The following items are required for admission to
Lowndes County Schools Kindergarten/Pre-Kindergarten program:

1. * Official birth certificate (no copies please)
2.  * Proof of residence (utility bill, cable bill, sanitation bill, mortgage contract,
rental agreement (signed by both parties).  NO phone bills .
Pre-K acceptance letters will be mailed following residency verification.
3.  * Completed Pre-K application packet
* These items are required at the time of registration
4.   Shot record on Georgia Certificate of Immunization  (Form 3231)
5.   Certificate of Eye, Ear, Dental Examination  (Form 3300)
6.   Copy of child's Social Security card
7.   Proof of income (Pre-K only), or
Proof of participation in following programs, IF APPLICABLE (Pre-K only)
Ø  SSI
Ø  Georgia Temporary Support Grant
Ø  Medicaid/Peachcare
Ø  Food Stamps
Ø  Temporary Assistance to Needy  Families (TANF)
